The purpose of this meeting is to conduct the governmental business outlined in the above agenda. The advisory council will provide strategic guidance and partner input to DHS as part of the state’s implementation of the Rural Health Transformation Program. The program is designed to strengthen rural health care systems and improve access, quality, workforce capacity, and care coordination across the state’s rural communities.
Wisconsin’s Rural Health Transformation Program is supported by the Centers for Medicare & Medicaid Services of the U.S. Department of Health and Human Services as part of a financial assistance award totaling $203,670,005.21 with 100 percent funded by CMS/HHS.
